Job Summary
Direct Support Professional provides individualized supports in the client’s own home or family homes, helping waiver participants develop independence through activities of daily living, personal care, life skills, community integration, and transportation navigation. The role emphasizes adhering to Person-Centered and Behavior Support Plans, documenting services in agency systems, promoting decision-making and self-help, and completing required trainings (CPR/First Aid, Mandt). The position requires reliable transportation, the ability to drive as part of daily duties, and ongoing coordination with the Operations team, with hours Monday and Friday evenings. No relocation or remote work is described; community-based, in-home support is central.
Required Qualifications
- High School Diploma or Equivalent (GED)
- Valid driver's license
- Reliable transportation
- 1-year driving record with no more than 3 points
- Ability to walk, stand, and sit for up to 8 hours
- CPR/First Aid certification
- Mandt Systems training (or willingness to complete)
- Experience or training in MTTP (preferred)
- Ability to lift up to 50 lbs (as needed)
- Ability to use agency electronic systems for time/attendance, progress notes, incident reports
