Job Summary
Empower supported individuals to live, work, and thrive in their chosen environments by assisting with personal care, daily living skills, and community transportation. Serve as a liaison to the community, developing and executing Personal Care Plans while reviewing Behavioral Plans and reporting abuse or neglect immediately. Complete daily documentation including Caretracker data, medication administration, and Incident Reports within 24 hours. Provide peer training for new team members and maintain all required certifications. Uphold the person-centered philosophy and agency policies while tracking time accurately and collaborating across departments. Maintain a valid driver's license and reliable transportation for community engagement.
Required Qualifications
- High School Diploma or equivalent
- Access to reliable transportation
- Active valid driver's license
- Clean driving record
- No more than two (2) points on driving record
- Proof of current automobile insurance
- Flexibility of scheduling to meet agency and program needs
Desired Qualifications
- Two (2) years' experience in a residential setting
- CNA/GNA Certification
- CPR and First Aid Certifications
