Job Summary
Conduct comprehensive benefit eligibility screenings, assessments, and appointments for individuals and families to connect them with necessary resources. Guide clients through application processes, renewals, and advocacy while maintaining strict HIPAA compliance and confidentiality. Coordinate community outreach events, distribute materials, and collaborate with local organizations to promote the program's mission. Track client data in the Charity Tracker system, report feedback to leadership, and provide outside referrals to ensure all household needs are met.
Required Qualifications
- Solid case management/program experience
- Organizational and time management skills including attention to detail
- Proficient computer skills required; ability to comfortably use internet-based applications, Microsoft applications
- Strong interpersonal/communication and leadership skills
- Ability to anticipate, coordinate detail, and work independently
- Professional business appearance and demeanor
- Knowledge of qualifications for entitlement programs and healthcare terminology
- Strong oral and written communication skills
- Knowledge of Montgomery County human service system and resources
- Experience and comfort in presenting program information to small and large groups
- Capability to participate in training sessions, presentations, and meetings
- Possess a valid state driver's license with a clean driving record
- Ability to handle high-stress situations on a day-to-day basis
- Ability to make judgment decisions and communicate clearly
- Dexterity of hands and fingers to operate keyboard, mouse, and other devices and objects
- Employees must be able to lift objects up to 25 lbs
