Job Summary
Provide direct hands-on personal care to clients in their homes as a PCSP II, including assistance with ambulation and mobility, bathing, dressing, grooming, personal hygiene, fall prevention, transfers, and light household tasks related to client care. Monitor and report changes in client condition to the care team while maintaining dignity, safety, and independence. Require comfort with physical caregiving and a calm, respectful, professional demeanor; ability to lift at least 25 pounds and perform related physical tasks. Must possess a valid driver’s license, reliable transportation, proof of auto insurance, pass criminal and driving background checks, complete onboarding and on-site training with the nurse manager, and have a smartphone with GPS for electronic visit verification.
Required Qualifications
- Minimum of 6 months experience in a home care, personal care, or similar setting (experience with older adults or individuals with disabilities preferred)
- Ability to pass criminal and driving background checks and a pre-employment physical, including TB screening
- Completion of required onboarding, competency training, and on-site training (OST) with the nurse manager
- Smartphone with GPS capability for electronic visit verification
- Ability to lift at least 25 pounds and perform physical tasks including bending, reaching, and stair navigation
- Valid driver’s license, reliable transportation, and proof of auto insurance ($50,000/$100,000 minimum)
- High school education preferred (may be waived based on relevant experience)
- Must be at least 18 years of age
