Personal Care Management Skills Trainer

Assess consumers and surrogates on their ability to manage Personal Care Attendant (PCA) services, then train them on hands-on care skills outlined in the Mass Health Service Agreement. Maintain confidentiality while documenting progress notes, intake referrals, and quarterly reviews to track required documentation within contract timelines. Return all consumer and surrogate phone calls within 72 business hours and submit internal expense and mileage forms promptly. Provide ongoing support for Electronic Verification of Visits (EVV) and address major problems requiring skills training. Report suspected abuse or neglect to the Disabled Person's Protection Commission immediately. This full-time role covers the Woburn area and a 60-mile radius, requiring a valid driver's license and reliable vehicle.
