Job Summary
RN-needed in a community-based Personal Care Management program; perform initial and annual PCA evaluations under MassHealth regulations, respond to evaluation requests, schedule reevaluations, document progress notes, and coordinate with PCA Eval Dept and supervisor; travel within MA coverage area to perform community visits; requires MA RN licensure, at least 2 years direct-care experience with disabled/elderly populations, strong communication and organizational skills, ability to work in a team, and flexibility for community-based duties.
Required Qualifications
- Current Massachusetts license to practice as a Registered Nurse
- 2 years recent experience in direct care of the elderly or disabled population
- Valid driver’s license and reliable transportation
