Job Summary
Lead and supervise Personal Care Attendants delivering ADL/IADL support for elderly participants in a homecare-based program; manage PCA schedules, training, and evaluations; collaborate with multidisciplinary teams to develop and monitor care plans; perform participant/home visits to coach PCA performance, ensure CMS and center policies are followed, and maintain documentation and reporting on care delivery and outcomes; requires strong organizational, supervisory, and communication skills, proficiency with Microsoft Office, and the ability to work in an indoor, community-based setting.
Required Qualifications
- High School Graduate or GED; Associate degree or equivalent relevant work experience; CPR certification; immunizations up-to-date
