Job Summary
Perform and assist with personal care ADLs/IADLs for participants in the PACE program across center, home, and virtual platforms. Support PCA III in gathering and reporting care coordination information regarding participant engagement, function, and socialization. Collaborate with IDT members and PACE staff on plan of care goals, addressing needs for supplies, tools, health status, and physical or mental well-being. Requires Nurse Assistant Certificate, BLS certification, and preferred bilingual skills.
Required Qualifications
- Certified Nursing Aide education and training
- Nurse Assistant Certificate by the State of California, Department of Health and Human Services-Aid and Technician Certification
- High school diploma
- Valid BLS certification or higher following the American Heart Association (AHA) or the American Red Cross guidelines
Desired Qualifications
- some college education/credits
- California Food Handler Certificate of Achievement
- Prior experience working with physically or mentally impaired older people
- Bilingual English/Spanish/Mandarin/Cantonese
- Experience working with the elderly patient population
