Personal Care Assistant

Job Summary
Conduct ongoing participant assessment and monitoring, reporting self-care abilities and service delivery. Provide personal care assistance for activities of daily living including toileting, showers, eating, and grooming. Assist with physical activity, exercise, and walking while identifying safety hazards and initiating corrective action. Support general activities such as conversing, recreation, meal serving, and housekeeping. Escort participants to appointments and assist with loading/unloading vehicles. Report signs of physical, emotional, or behavioral changes to staff. Work one weekend monthly with rotating holiday coverage, requiring a valid driver's license and use of a personal vehicle for home care visits. Position involves occasional facility work and up to 10% regional travel.
Required Qualifications
- High School Diploma or equivalent
- Minimum of one (1) year of documented experience working with a frail or elderly population
- Experience in a long-term care facility or with a community-based geriatric program preferred
- Must be action-oriented
- Manage conflict well
- Be customer focused
- Have high decision quality
- Flexibility to adapt to ongoing change
- Ability to work with minimal supervision
- Exercise independent judgment
- Superior written and verbal communication and presentation skills appropriate for audience comprehension
- Valid drivers license
- Use of personal vehicle for home care visits
- Excellent organizational skills
- Ability to perform multiple duties and functions related to daily operations while maintaining excellent customer service
- Must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring (if less than one (1) year of experience)
