Job Summary
Conduct monthly home visits for program members and their Care Providers to ensure well-being and regulatory compliance. Complete all necessary documentation accurately to maintain thorough records of each visit. Maintain open lines of communication with assigned staff and coordinate with external service providers to ensure comprehensive care. Make appropriate referrals to external resources when additional support is needed and monitor home settings according to guidelines. Requires a Bachelor's degree in social work or human services, valid driver's license, and strong interpersonal skills. Schedule is Monday-Friday 8am-4pm with flexibility needed for program needs.
Required Qualifications
- Must pass background checks and pre-employment screenings including TB test and CORI
- Bachelor's degree in social work, human services, or a related field (or equivalent experience)
- Basic computer skills using Microsoft Office, email communication, internet, and documentation entry
- Strong interpersonal and communication skills to effectively collaborate with program participants, their Care Providers, and a multidisciplinary team
- Ability to prioritize tasks, work independently and within a team, and meet deadlines
- Compassionate, resourceful, and eager to learn
- Valid driver's license and reliable personal vehicles meeting state requirements
- Must be available for Monday-Friday 8am-4pm schedule
- Must be available for 40 hr/week
- Must be flexible with schedule to meet programs needs
