Job Summary
Permanency Liaison responsible for overseeing all case management tasks related to children exiting out-of-home care and achieving legal permanency (reunification, adoption, or guardianship). Duties include drafting and coordinating documents for permanency processes (RPR/ROG referrals, TPR referrals), ensuring required documents are submitted to court, reviewing adoptive resources/guardianship options, attending home visits to discuss permanency, recommending legal permanency options, collaborating with child welfare colleagues, and managing timelines across multiple priorities. Requires a Bachelor’s Degree in Social Work (BSW) with Wisconsin state certification preferred within 18 months; hybrid on-site/remote work in Milwaukee, WI with evenings and some weekends to accommodate program needs; PSLF eligibility consideration; strong communication, data/documentation accuracy, and proficiency with computer software (Microsoft Office, Excel, Outlook, Teams, ADP).
Required Qualifications
- Bachelor's Degree in Social Work (BSW) from an accredited program required
- Preferred state certification as a Social Worker in Wisconsin within 18 months of hire for BSWs
- Preferred that Non-BSWs complete approved training within 24 months and state certification by 36 months of hire
- Experience in Child Welfare or related human services field preferred
- Proficiency with Microsoft Office, Excel, Outlook, Teams, ADP
