Job Summary
Permanency Case ManagerResponsibilities include managing a caseload of children in the Managing Conservatorship of DFPS, assessing needs and safety, facilitating permanency planning and safe placements, conducting ongoing kinship searches, monthly visits with children and families, post-reunification support until court dismissal, coordinating with public/private agencies, preparing assessments and court reports, providing testimony, maintaining case documentation, training families and conducting home visits, collaborating with Adoption Coordinators to prepare children for adoption, participating in 24/7 on-call rotation to respond to crises, working with Care Management and PAL staff to support placements and older youth, compiling monthly reports, and promoting cultural diversity. Requires a Bachelor’s Degree and offers competitive salary, benefits, and a comprehensive package; drive with personal or agency vehicles as needed.
