Permanency Case Manager

Manage a caseload and guide families throughout the permanency process by assessing child and family needs, safety concerns, and strengths. Facilitate permanency planning, conduct ongoing searches for Kinship placements, and visit children and families monthly or more often as needed. Provide post-reunification support until court dismissal, partner with public and private agencies to coordinate services, and prepare assessments, court reports, and testimony. Maintain complete case documentation, prepare families through training and home visits, and collaborate with Adoption Coordinators. Participate in 24/7 on-call rotation to respond to crises, work with Care Management and PAL staff, and compile monthly reports. Use personal or agency vehicles to transport children safely while championing cultural diversity and sensitivity.
