Perinatal Nurse Coordinator, Access Health Lakelands

Job Summary
Identify and manage high-risk patients, conduct clinical chart audits, and support remote patient monitoring. Provide direct patient education and coordinate care for expectant mothers, including home visits to assess needs and deliver clinical support. Collaborate within a multidisciplinary team to address medical and social needs, ensuring comprehensive care across Self Regional Healthcare's service area. Collect, analyze, and report patient data for program evaluation while utilizing electronic health records, preferably EPIC. Full-time, first shift Monday-Friday 8:00-4:30. Current LPN or RN license in South Carolina required with minimum two years of maternal health experience.
Required Qualifications
- Current LPN or RN license in South Carolina
- Minimum of two years of experience in maternal health, OB/GYN, community health, or chronic disease management
- Familiarity with managing patients with hypertension, diabetes, and high-risk pregnancy conditions
- Experience with electronic health records (EHR), preferably EPIC
- Ability to collect, analyze, and report patient data for program evaluation
- Strong communication and patient education skills
- Ability to work collaboratively with interdisciplinary teams
- Cultural competency and experience working with diverse populations
Desired Qualifications
- Certification in Care Management (CCM) or Chronic Disease Management
- Experience with remote patient monitoring (RPM) technologies
- Prior experience working with community-based health programs or public health initiatives
- Bilingual (Spanish/English)
