Job Summary
Conduct wound assessments and provide personalized treatment plans for patients in skilled nursing, long-term care, and rehabilitation facilities. Collaborate with healthcare professionals to ensure optimal care, complete documentation accurately, and educate patients on wound care and prevention. Monitor patient progress and adjust treatment plans as needed. This role offers $150-$200 per hour with no call, no weekends, and no hospital rounding responsibilities. Integrated Wound Care develops services that reduce wound-related hospitalizations and improve patient outcomes through efficiencies in costs and care delivery.
Required Qualifications
- Active Nurse Practitioner license in state of employment
- Minimum of 1 year of experience in wound care
- Ability to work independently and as part of a team
- Exceptional critical thinking and decision-making skills
- Excellent verbal and written communication skills
- Strong time management and organizational abilities
- Comfortable with technology and electronic medical records
