Job Summary
Per Diem Speech Language Pathologist (SLP) needed for field-based Home Care. Responsibilities include conducting evaluative procedures (including swallowing evaluations), identifying equipment needs related to speech-language function, establishing goals and a plan of care to improve patient function, administering speech therapy programs with appropriate techniques/equipment, and maintaining compliance with agency documentation standards via the EMR. Includes submitting clinical documentation and time records in a timely manner. Required Qualifications
- Licensed and Registered Speech Language Pathologist (SLP) in the state of employment
- At least 1 year of clinical experience; Home Care experience preferred
- Oasis experience strongly desired
- Have reliable transportation and willing to commute to provide the best in-home patient care
