Job Summary
Conduct initial psychosocial evaluations, ongoing counseling, and direct casework for hospice patients and families. Assess psychosocial status, family dynamics, and caregiver abilities to plan interventions, while providing crisis intervention and bereavement services. Educate interdisciplinary teams on cultural needs and assist with advanced directives, discharge planning, and funeral arrangements. Serve as a liaison to community agencies and maintain clinical records with timely Point of Service documentation. Participate in interdisciplinary meetings and plan of care development as needed.
