Job Summary
Conduct thorough psychosocial assessments of patients in relation to their illness and environment, communicating findings to the multidisciplinary home health team. Carry out social evaluations, develop intervention plans, and maintain accurate clinical records to ensure continuity of care. Act as a liaison between patients, families, and community agencies to leverage resources, while assisting physicians in understanding social factors impacting health. Participate in case conferences and discharge planning to facilitate seamless patient transitions. Requires a Master's degree in Social Work, valid LICSW license, and one year of healthcare experience. Per diem rate with mileage reimbursement; based in York, ME.
Required Qualifications
- Holds a master's degree from a school of social work accredited by the Council on Social Work Education
- Holds a valid license as an LICSW in good standing
- Possesses a minimum of one (1) years' experience in health care
- Demonstrates excellent verbal and written communication skills
- Maintain current BLS CPR certification
- Possess a valid driver's license
- Own a reliable, insured vehicle
- Possess a valid driver's license and own a reliable, insured vehicle
