Job Summary
Conduct skilled nursing assessments, oversee medication administration, and manage wound care for adult and geriatric patients in home, assisted living, and skilled nursing facilities. Provide visiting home health services including admissions, re-certification, and discharges while developing coordinated plans of care with healthcare professionals. Educate patients and caregivers on disease processes and treatment plans, then report daily to the Clinical Manager and document care using an iPad within required timeframes. Requires RN licensure with 1-2 years of experience, reliable transportation, and willingness to work flexible per diem hours including weekends and holidays.
Required Qualifications
- Registered Nurse
- Licensed in the state of employment
- graduate of an accredited RN program
- At least 1-2 years of RN experience
- Have reliable transportation
- willing to commute to provide the best in-home patient care
- Some weekend days
- summer and winter holiday commitments
