Per Diem RN Home Health Admissions/Float

Conduct comprehensive patient assessments and develop care plans in collaboration with patients and families across private residences, adult family homes, and assisted living facilities. Perform admission tasks including education, resource coordination, physician communication, pain management, and skilled physical therapy interventions. Expand OASIS knowledge and expertise while documenting charting and communication using computer skills. Requires an active WA RN license, valid driver's license, and strong OASIS experience. Home Health experience is strongly preferred, along with one year in admissions.
