Job Summary
Conduct clinical admissions for all Company services by explaining services to potential patients, identifying entry barriers, and completing informed consents and initial assessments. Initiate skilled nursing interventions to prevent complications, obtain physician orders, and develop a Plan of Care to address immediate needs. Act as the Company representative at assigned facilities by facilitating referrals, communicating with Care Navigation staff, and discussing potential needs with after-hours personnel. Provide care for GIP patients in hospitals through daily visits, professional management, and efficient discharge while maintaining confidentiality and complying with all regulations.
Required Qualifications
- Current license as RN in the state where the employee will be working
- Active BLS for healthcare professionals from the American Heart Association or Red Cross
- Minimum of one (1) year of RN experience
- Strong clinical assessment skills
- Ability to effectively communicate in English (verbal and written)
- Valid driver's license
- Automobile insurance per Company policy
- Reliable transportation to meet schedule
- Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation

Desired Qualifications
- Hospice or hospital experience
- Background in emergency department or ICU
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
- Bi-lingual in Spanish
- Ability to present services in a professional and articulate manner
- Ability to work in a team environment and intervene appropriately
- Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload
