Job Summary
Conduct clinical triage, direct patient care, and medication management in an ambulatory care setting. Perform wound assessments, administer medications, and support Office-Based Addiction Treatment (OBAT) programs as needed. Manage transitional care by conducting post-discharge outreach, reviewing discharge instructions, and coordinating follow-up appointments. Monitor electronic medical record messages for urgent needs, process refill requests, and ensure accurate documentation in compliance with regulatory requirements. Collaborate with providers, pharmacies, and community agencies to improve patient outcomes and ensure continuity of care. This flexible per diem role supports the Nursing Services Department at North Olympic Healthcare Network in a fast-paced community health center environment.
Required Qualifications
- Graduate of an accredited school of nursing
- Current, active WA State Registered Nurse license
- Certified Basic Life Support
- Proficiency in Electronic Health Record systems
- Ability to utilize computers for data entry and information retrieval
- Excellent verbal and written communication skills
- High level of skill at building relationships and customer service
- Influential skills in managing difficult clients and patients
- High degree of knowledge and competency in the practice of medicine and associate charting requirements
- Requisite skills and ability to perform certain medical tasks as assigned
