Per Diem Registered Nurse (RN)- Home Health

Provide comprehensive skilled nursing visits in patients' homes, perform assessments, and develop individualized care plans. Complete Start of Care, Resumption of Care, Recertification, Routine, and Discharge visits while accurately documenting OASIS assessments and clinical information. Educate patients and caregivers on disease management, medications, and safety, then coordinate care with physicians and the interdisciplinary team. Monitor patient progress and promptly report condition changes, ensuring compliance with Medicare, CMS, and agency policies. Maintain accurate electronic documentation and participate in case conferences as needed.
