Job Summary
Conduct comprehensive physical assessments and history reviews for patients and families to determine hospice needs. Write and initiate plans of care, regularly re-evaluating status and revising protocols as patient conditions change. Document pain/symptom management outcomes, teach self-care techniques, and communicate with physicians regarding orders and condition changes. Collaborate with the interdisciplinary group and include patients in the planning process. Requires one year of acute care experience, current licensure, and flexibility in work hours. Coverage spans Tremonton to Bountiful.
Required Qualifications
- Registered Nurse (RN)
- at least one (1) year of acute care experience in an institutional setting
- current licensure in the state
- Must understand the philosophy of hospice concept and understand needs of the terminally ill
- Must be flexible in work hours
- Must demonstrate nursing skills per competency checklist
- Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
- Ability to travel throughout the Bristol Hospice Service Area
Desired Qualifications
- One (1) year of experience in home care preferred
