Job Summary
Conduct comprehensive assessments of patients and families to determine hospice needs, including physical histories and status evaluations. Initiate and revise nursing care plans based on diagnoses, while documenting pain management and communicating condition changes to physicians. Teach self-care techniques to patients and caregivers, collaborating with the interdisciplinary group to deliver family-centered hospice services. Requires one year of acute care experience, current licensure, and flexibility for per diem hours across the Tremonton to Bountiful coverage area.
Required Qualifications
- Registered Nurse (RN)
- at least one (1) year of acute care experience in an institutional setting
- current licensure in the state
- Must understand the philosophy of hospice concept and understand needs of the terminally ill
- Must be flexible in work hours
- Must demonstrate nursing skills per competency checklist
- Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
- Ability to travel throughout the Bristol Hospice Service Area
Desired Qualifications
- One (1) year of experience in home care preferred
