Per Diem PT - Physical Therapist

Conduct direct, consultative, and supportive physical therapy services, including evaluations, treatment planning, and interventions, in compliance with APTA standards, state regulations, HIPAA, and organizational policies. Perform assessments to determine individual needs, functional abilities, and rehabilitation goals within residential settings, then develop and modify individualized treatment plans to promote independence and daily living skills. Deliver individual, group, and consultative therapy while recommending, demonstrating, and training individuals and staff on adaptive equipment and positioning techniques. Collaborate with interdisciplinary teams, families, and program staff to ensure coordinated care, and actively participate in team meetings to communicate assessment results and treatment outcomes. Maintain accurate documentation of evaluations, progress notes, and treatment summaries in accordance with regulatory requirements.
