Job Summary
Clinical Architect & Patient Advocate leading weekday home-based RN care as a per diem private duty RN for Assured Quality Homecare. You will conduct comprehensive home evaluations and initiate new cases during standard business hours, provide clinical oversight for the CNA team, collaborate with physicians, and lead hospital discharge planning for seamless transitions. Manage complex nursing tasks including advanced wound care and medication administration, and document in EMR to keep DON and families aligned. Requires 5+ years of RN experience with an active RI RN license, CPR certification, a valid driver’s license, and an insured vehicle. This role offers a flexible weekday schedule with per diem availability and focuses on outpatient homecare rather than hospital shifts.
Required Qualifications

- 5+ years of RN experience (Rehab, Long-Term Care, or Assisted Living preferred)
- 5+ years of experience in client care planning and navigation
- Knowledge or experience with case management

- Proficient in EMR software
- Strong clinical decision-making in home setting
