Job Summary
Conduct home physical therapy visits including evaluations, start of care, follow-ups, discharges, and OASIS assessments. Assess patient vitals, determine treatment plans, and instruct patients on therapeutic exercises, gait training, and safety protocols to improve daily living activities. Educate patients and families on durable medical equipment use and design home exercise programs for post-discharge continuity. Document patient progress, coordinate with physicians and support staff, and ensure compliance with infection-control policies and legal requirements. Maintain equipment through preventive maintenance and troubleshooting. Requires a valid Physical Therapy license in Massachusetts, current CPR certification, and home care experience.
Required Qualifications
- Degree in Physical Therapy from an accredited university
- Valid license to practice Physical Therapy within the state of Massachusetts
- Current CPR Certification
- Valid Driver's License
- Home care experience
- Frequently required to stand
- Frequently required to walk
- Frequently required to sit
- Frequently required to use hands and fingers
- Frequently required to climb, balance, bend, stoop, kneel or crawl
- Occasionally required to lift/push light weights (less than 25 pounds)
- Occasionally required to lift/push light weights (greater than 25 pounds)
- Moving, lifting, or transferring of patients may be required on occasion
- Frequent exposure to bloodborne and airborne pathogens or infectious materials
