Job Summary
Assess and evaluate patients' therapeutic status, home environments, and rehabilitation potential including muscle strength, mobility, and gait. Initiate individualized physical therapy programs while instructing families and caregivers on therapeutic appliances and prosthetic devices. Actively participate in case conferences, discharge planning, and peer consultation processes to ensure coordinated care. Prepare clinical progress summaries based on goal achievement and physician plans of care. Maintain current BLS CPR certification and hold a valid driver's license with an insured vehicle.
Required Qualifications
- Possess a degree as a physical therapist approved by an accredited organization
- Maintain current BLS CPR certification
- Possess a valid driver's license
- Own a reliable, insured vehicle
Desired Qualifications
- A minimum of two years of relevant experience as a physical therapist
- Community/home health experience
