Per Diem Physical Therapist - Home Health

Deliver physical therapy services in accordance with a physician's plan of care and the state Physical Therapy Practice Act. Assess and evaluate therapeutic, rehabilitative, and functional status, including home environment assessment, muscle strength, mobility, gait, and range of motion. Administer treatments, instruct patients and caregivers on therapeutic appliances, and offer prosthetic device training. Initiate and manage programs by collaborating with healthcare professionals and family members, while reporting patient reactions and condition changes. Contribute to discharge planning, prepare clinical summaries, and conduct in-service education for nursing personnel. Supervise physical therapy assistants and evaluate home health aide care when therapy is the sole skilled service.
