Job Summary
Per Diem Orthopedic Home Physical Therapist in Hudson County, focusing on orthopedic rehabilitation in patients’ homes. Responsibilities include evaluating and treating patients in the home setting, executing the plan of care, and preparing for potential transition to JAG outpatient orthopedic locations for continuity of care. Responsibilities also cover educating patients on safe bed mobility, transfers, ambulation, ADLs/IADLs, documentation using Prompt EMR, collaboration with interdisciplinary team, and supporting billing/code processes. Requires licensure as a Physical Therapist, state CEU completion, CPR certification, and strong communication, teamwork, and clinical decision-making skills. Required Qualifications
- Bachelor’s or Master’s Degree in Physical Therapy
- Licensed Physical Therapist in NY/NJ
- Completed state CEU requirements (NY, NJ)
- Valid CPR License
- EMR experience preferred
- Strong communication and teamwork
- Bachelor’s or Master’s degree required
- Doctorate preferred (education listing shows Doctorate or better as preferred)
