Job Summary
Evaluate and treat patients recovering from total hip and knee replacement surgeries in their home setting, focusing on safe bed mobility, transfers, and ambulation. Collect and record patient care data, including initial evaluations, daily notes, and discharge plans, while completing daily billing. Establish treatment plans based on assessment results and provide education on therapeutic activities and community reintegration. Collaborate with interdisciplinary teams to modify care goals and ensure compliance with department policies. Communicate with the Manager and Patient Access team for scheduling evaluations once demographics are received.
Required Qualifications
- Bachelor's or Master's Degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Strong orthopedic background with specialization in Total Hip and Total Knee replacement rehabilitation
- Masters or better
- Bachelors or better
