Job Summary
Provide skilled occupational therapy in accordance with the physician's plan of care, including evaluating and re-evaluating client status. Initiate, develop, implement, and revise the client's plan of care (POC) while addressing all identified problems. Coordinate and oversee intermittent care to clients, instructing caregivers to promote maximal functional status in the home. Reassess clients following hospital discharge or condition changes and communicate with other disciplines as warranted. Deliver educational information to clients, families, and caregivers regarding knowledge deficits. Work independently under pressure with heavy physical demands involving lifting up to 100 pounds occasionally. Maintain current CPR/BLS certification, hold a valid driver's license, and possess reliable transportation.
Required Qualifications
- Registered by the American Occupational Therapy Association or successfully completed the educational requirements of occupational therapy and licensed in the state
- Minimum of one (1) year of occupational therapy experience
- Current CPR/BLS
- Reliable transportation
- current Driver's License

- Heavy physical demand level
- Lift / Carry 50 to 100 pounds occasionally, 25 to 50 pounds frequently, 10 to 20 pounds constantly
- Sit, Stand, Walk, Bend, Squat, Kneel, Crawl, Climb and Reach above shoulders
- Push and Pull
- Perform fine motor functions
- Possess functional vision, hearing, and speech to communicate effectively with clients, client advocates, and staff
- Coordinate multiple tasks simultaneously
- Collect, interpret, and/or analyze complex data and information
- Understand and respond to a diverse population
- Communicate electronically
