Job Summary
Provide nurse visits to home health patients by implementing RN-initiated plans of care and delivering basic treatments to ill, injured, or convalescing individuals. Communicate with patients to schedule visits and educate them on self-care during non-visit days while documenting consistently with the care plan. Support the Director of Nursing, Case Management, and nursing staff in delivering health care within a home setting, ensuring full compliance with regulatory requirements. Requires a current CA LVN license, one year of home health experience, and BLS certification.
Required Qualifications
- Clinically experienced nurse
- Nursing documentation/charting skills
- Fast learner
- Great communication skills
- Highly motivated
- Positive-minded
- Visiting LVN for Home Health: 1 year
- Nursing: 1 year
- Current CA LVN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- Ability to work at least two-three days each week (or ten productivity units)
- Ability to work one weekend a month
- Ability to work two holidays per year (one winter and one summer)
- Knowledge of regulatory requirements
Desired Qualifications
- IV/Phlebotomy Certified
