Job Summary
Plan and deliver safe, age-appropriate patient care for chronic dialysis patients, incorporating family-centered practices and multidisciplinary team collaboration. Evaluate clinical status, document responses to interventions, and modify care plans to optimize outcomes while addressing patient satisfaction and conflict resolution. Process medication orders, verify dosages, obtain prior authorizations, and coordinate referrals for tests. Maintain exam room cleanliness, ensure emergency equipment is operational, and escalate issues per policy. Complete required documentation, advance care directive assessments, and intra/inter-shift reporting.
Required Qualifications
- Dialysis experience as an LPN
- High School or Equivalent Technical Diploma
- Graduate of accredited school of licensed practical nursing
- 0 - 1 year nursing experience working in a hospital, ambulatory, private practice or home health care setting
