Job Summary
Provide direct patient care under registered nurse supervision by implementing plans of care and delivering treatments. Deliver accurate documentation, educate patients and families on diagnosis and care plans, and coordinate home health services by reporting needs to the clinical supervisor. Utilize equipment and supplies effectively while participating in personal and professional growth. Maintain current CPR certification and hold a valid driver's license with an insured vehicle. Requires three years of nursing experience including one year in home health care.
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Three years nursing experience
- One year visiting home health care experience
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state
- Compliance with accepted professional standards and practice
- Good verbal and written communication skills
- Organization skills
- Licensed driver status
- Automobile insured in accordance with state and organization requirements
- Automobile in good working order
- Current CPR certification
