Job Summary
Per Diem Licensed Practical Nurse (LPN) providing end-of-life hospice nursing in patients' homes and communities under the guidance of an RN. You will administer treatments, implement care plans, maintain accurate documentation, and educate patients and families; collaborate with interdisciplinary teams (RNs, hospice aides, social workers, and others); participate in Hospice IDT meetings; manage pain and symptoms; and contribute to quality improvement efforts while supporting a compassionate, family-centered approach.
Required Qualifications
- Graduate of an accredited practical nurse program.
- LPN licensed in the state of employment.
- Preferably, experience in hospice, palliative or community health home care or medical/surgical settings.
- Background in clinical auditing and chart reviews is a plus.
- Wound care experience is preferred.
- Understanding of hospice philosophy and the needs of the terminally ill.
