Job Summary
Conduct home health patient visits by performing assessments, implementing RN-initiated plans of care, and providing basic treatment. Communicate with patients to schedule appointments and educate them on self-care during non-visit days. Document all care consistently with the established plan while demonstrating knowledge of regulatory requirements. Support the Director of Nursing and nursing staff in delivering health care services to ill, injured, or convalescing patients. Work as a per diem LVN with flexible scheduling, including weekend and holiday coverage, earning $45.00 per visit.
Required Qualifications
- Clinically experienced nurse
- Nursing documentation/charting skills
- Fast learner
- Great communication skills
- Highly motivated
- Positive-minded
- Visiting LVN for Home Health: 1 year
- Nursing: 1 year
- Current CA LVN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- Available to work at least two-three days each week
- Available to work one weekend a month
- Available to work two holidays per year
