Job Summary
Review and clinically assess incoming referrals to determine appropriateness for home health services, ensuring accurate and timely processing while maintaining compliance with agency, Medicare, and regulatory requirements. Collaborate with Clinical Managers and Intake Coordinators to coordinate daily admissions, prioritize referrals, and manage agency capacity by communicating with physicians, hospitals, and case managers to obtain clinical documentation. Build strong relationships with referral partners through education and support, serving as a clinical resource to triage referrals using sound clinical judgment. Maintain accurate documentation within the electronic medical record and work independently in a remote environment. Requires current Massachusetts RN or LPN license, 3+ years nursing experience, and Medicare/Managed Care expertise. Schedule includes minimum 2 shifts monthly during 8:00 AM to 5:30 PM day shifts.
Required Qualifications
- Current Massachusetts RN or LPN license in good standing
- Minimum of 3 years of nursing experience
- Medicare and Managed Care experience
- Working knowledge of home health and hospice regulations
- Strong clinical assessment, critical thinking, and triage skills
- Excellent communication, interpersonal, and relationship-building abilities
- Proven ability to prioritize multiple referrals in a fast-paced environment
- Strong organizational skills with exceptional attention to detail
- Ability to work independently in a remote environment while collaborating effectively with an interdisciplinary team
- Remote work must be done in Massachusetts
- Minimum commitment of 2 shifts per month
- Department operational hours are 7 days per week, with day shifts between 8:00 AM and 5:30 PM
Desired Qualifications
- 1+ year of direct home health experience
- Experience with electronic medical records; MatrixCare experience is preferred
