Job Summary
Initiate preventive and rehabilitative nursing procedures, administer prescribed medications, and conduct initial patient and family assessments to determine home care needs. Perform thorough physical assessments, develop individualized plans of care, and collaborate with physicians and interdisciplinary teams to create robust treatment plans. Responsively address inquiries from patients and caregivers, provide counseling support before, during, and after treatment, and supervise home health aides as required. Identify discharge planning needs and proactively implement them within the care plan. Requires an active nursing license, geriatric experience, and valid driver's license. Based in Londonderry, NH, serving Manchester and surrounding areas with a per diem schedule.
Required Qualifications
- Must hold an active Nursing license in the State where the position is based.
- Nursing degree from an accredited institution.
- Maintain current BLS CPR certification.
- Possess a valid driver's license and own a reliable, insured vehicle.
- Working knowledge of healthcare laws and regulations.
Desired Qualifications
- Preferably 2+ years of nursing experience, with a focus on geriatrics.
- Excellent bedside manners and exceptional communication skills.
