Per Diem Hospice Registered Nurse

Job Summary
Provide in-home hospice nursing care, administer medications and treatments, conduct initial and ongoing assessments, develop and revise Plans of Care, collaborate with physicians and interdisciplinary teams, address inquiries from patients and caregivers, provide counseling and support, supervise home health aides, and identify discharge planning needs as part of the care plan. Must hold an active nursing license in NH, prefer 2+ years in geriatrics, and possess a nursing degree with BLS CPR certification; valid driver’s license and reliable vehicle are required. Per visit compensation available with mileage reimbursement and flexible scheduling.
Required Qualifications
- Active Nursing license in the state where the position is based
- Nursing degree from an accredited institution
- BLS CPR certification
- Valid driver's license and own vehicle
