Per Diem Home Health Speech Therapist

Field-based speech-language pathology role providing assessment, treatment planning, and therapy for patients served in home health across Contra Costa County. Responsibilities include evaluating patient needs, identifying equipment and devices as indicated, administering speech therapy, coordinating with referring physicians and other staff to ensure a comprehensive care plan, participating in meetings/case conferences, preparing progress notes, maintaining clinical competency, identifying patient and family needs for additional services, instructing patient/family/other health care personnel in treatment regimes, and potentially supervising a home health aide. Requires a Master’s degree in Speech–Language Pathology, ASHA CCC, active licensure, CPR certification, and at least 1 year of clinical experience in Speech–Language Pathology. Demonstrates strong communication and organizational skills and access to ongoing professional development and supportive administrative resources.
