Per Diem Home Health Registered Nurse (Medicare Home Health Visits) Jupitar FL,

Provide safe, effective, and patient-centered nursing care to Medicare home health patients in accordance with individualized Plans of Care. Accurately complete and document all visits in the EMR, including Start of Care and ongoing assessments using the OASIS tool. Deliver skilled nursing services including medication administration, wound care, infusion therapy, catheter care, and safe patient positioning. Perform required supervisory visits and communicate promptly with the interdisciplinary care team regarding patient changes or emergencies. Maintain professionalism, empathy, and strong clinical judgment while collaborating with Nurse Supervisors to ensure continuity of care. Requires active Florida RN license, Medicare home health experience, OASIS proficiency, and wound care expertise.
