Per Diem Home Health Medical Social Worker

Per Diem Medical Social Worker role providing in-home visits in Worcester, Auburn, Charlton, Uxbridge, and surrounding towns. Responsibilities include conducting home visits, performing clinical social evaluations, planning interventions, identifying community resource referrals, advocating for clients and families, collaborating with a multidisciplinary team, developing and maintaining contracts with public/private agencies, educating patients and families about community resources to promote health and independence, and documenting care using a state-of-the-art touch pad tablet. Credentials required: MSW and LICSW/LCSW licensed in MA. Per diem schedule with daytime visits (8:30am–5:00pm).
