Per Diem Home Health Medical Social Worker

Conduct home visits to clients in Worcester, Auburn, Charlton, Uxbridge, and surrounding towns to provide clinical social evaluations and plan interventions. Identify community resource referrals to address practical and environmental needs, acting as an advocate to navigate the community system. Participate in developing total plans of care with the multidisciplinary team and develop contracts with public and private agencies. Educate patients and families on community resources to promote health, safety, and independent living, while accurately documenting observations and evaluations using a touch pad tablet. Schedule weekdays between 8:30am and 5:00pm.
