Job Summary
Per Diem Home Health LVN provides patient- and family-centered nursing in the home across Santa Clara County (Mountain View, Sunnyvale, Santa Clara, San Jose, Palo Alto). The LVN works as part of a multidisciplinary team to assess patient needs, develop and carry out care plans, initiate preventive and rehabilitative nursing procedures, document nursing notes, liaise with physicians, and communicate changes promptly. Requires a minimum of one year of home health or community health experience and an active LVN license. Required Qualifications
- One year of experience in a home health agency or community health setting
- Licensed Vocational Nurse (LVN) license
- Knowledge of Medicare, Medi-Cal, State and Federal regulations
- Ability to assess and develop a plan of care per physician orders and agency policies
- Ability to communicate clearly with patient, supervisor and physician
- Ability to complete and submit time sheets and nursing notes per agency policy
- Clinical knowledge and critical thinking to coordinate patient care
- Proficiency in documentation and DME support
- Fexibility to work per diem home health assignments
- Ability to counsel and instruct patient and family to meet patient needs
- Strong interpersonal and customer service skills
