Job Summary
Provide skilled nursing visits and conduct comprehensive in-home patient assessments, including Start of Care evaluations, throughout Greeley and Northglenn. Accurately evaluate patients' medical conditions and functional status, then develop and update care plans in accordance with agency policies and regulatory requirements. Collaborate with physicians, Home Health Aides, and the interdisciplinary team to coordinate services, while educating patients and caregivers on disease management and safety. Complete all required clinical documentation and report significant changes in patient condition to clinical personnel. Ensure compliance with all state, federal, and Department of Health regulations while delivering compassionate, patient-centered care. This per diem role offers flexible scheduling and fee-for-service compensation, with weekly pay via direct deposit.
Required Qualifications
- Active Colorado Registered Nurse (RN) license in good standing
- Ability to work independently and effectively manage a field-based schedule
- Willingness to travel throughout Colorado
Desired Qualifications
- Minimum one (1) year of nursing experience
- Home care experience
- Strong clinical assessment, critical thinking, and documentation skills
- Excellent interpersonal, communication, and organizational skills
