Job Summary
Conduct clinical assessments, develop care plans, and coordinate interventions to support participants' nutritional health and manage chronic conditions per PACE policy and evidence-based guidelines. Complete nutrition assessments, progress notes, and care plan updates at enrollment, annually, and during status changes. Provide culturally and linguistically appropriate nutrition counseling to participants and caregivers while overseeing nutrition-related competencies for Dietary Assistants and Health Aides. Address high-risk issues including weight loss and skin breakdown through participant-centered goals and team collaboration.
Required Qualifications
- BS or MS
- At least one year as a Registered Dietitian
- At least one year experience with elders
- Registered Dietitian (RD/RL) or RD eligible from an accredited Academic institution
- ServSafe Safety Certification
- Massachusetts Allergen Training Certification
- Obtains state licensure within 3 months of hire
- Maintains State and National registration/licensure
- Maintains the American Dietetic Association Professional Portfolio
- Must complete a background check
