Job Summary
Coordinate outpatient counseling by scheduling patients, reviewing medical records, and assessing nutritional status through history, lab parameters, and eating habit analysis. Interview patients and families to determine knowledge levels, then provide education, clarify supplement order discrepancies, and recommend changes to physicians' orders. Collaborate with physicians, nurses, and other providers to implement care plans, manage TPN and tube feeding protocols, and assist in discharge planning. Document all interactions and adherence in the EMR, ensure referral forms are obtained, and participate in quality improvement projects and interdisciplinary committees.
Required Qualifications
- Bachelor's Degree in Dietetics or Nutrition
- Certified Dietitian-Nutritionist with State of Connecticut or certification eligible
- Registered Dietitian (R.D.) by the Commission on Dietetic Registration, Academy of Nutrition and Dietetics
- Ability to read and write in English
- Proficient in MNT, nutrition and disease management
- Knowledge of enteral formulas and TPN selection and management
- Ability to provide clear and effective verbal and written communication
- Ability to make accurate decisions based on clinical knowledge
- Ability to converse with other medical professionals
- Ability to develop and deliver presentations on nutrition topics

Desired Qualifications
- Minimum of 1 year of clinical or outpatient experience
