Job Summary
Provide intensive one-on-one support to youth and families through recreational, educational, and social activities, including role modeling in the home and community. Assist with homework, participate in treatment plan development, and conduct family team meetings to facilitate ongoing decision-making. Act as an advocate by linking families to community resources and maintain regular communication with guardians while providing transportation as needed. Document progress on educational, vocational, and behavioral goals, and collaborate with service providers like DSS and mental health clinics. Work flexible hours including evenings, weekends, and on-call rotations with a valid driver's license.
Required Qualifications
- Minimum High School diploma
- Valid Driver's License
- Acceptable driving history to the Agency
- Reliable car
- Ability to work a flexible schedule including various hours, nights, weekends and on call hours
- Travel
